how do I add a form of payment on site by credit card?

Hi,

You could use the existing payment methods (Admin -> Extensions -> Payments and if you want, you could install another payment type:

http://www.opencart.com/index.php?route=extension/extension&path=3&sort=e.date_modified&order=DESC
