Have updated everything and even uploaded the following patch fix you mentioned here:

With no luck we are at a loss, our customers are complaining about not being able to check out and we have resorted to losing some, and going back to emails and cheques…
Please help. This is what’s happening:
1. Unregistered users can not checkout (on clicking “Checkout” there is no option to enter any details (see screenshot)
2. Returning customers can not log in and then check out, log in form username and password field display but the submit button is missing.
