I was looking at http://www.now-vaping.co.uk built with Pavillion and see a simple form in the center of the HOME page and was wondering how the FORM was built? (and added)

Hi Frank,

What is most certain, is that this form is a built-in theme feature. You will need to use some third party extension to insert a custom form:
