I am wanting to add some further questions for the customer to fill out at the checkout stage, if possible I would like them to be able:

To select a delivery date (possibly with a calendar widget),
A ‘How did you hear about us’ dropdown
Two Radio button questions with several different answers
A question with a large text box for additional order details
