How do you add a questionnaire to the registration form. For example, I want a retailer to answer a few questions first before I approve them and be able to keep their answers somewhere in the database so I can view it at any time.

Unfortunately, this is not trivial modification and could not be covered by the free support offered by this forum. You may post your question in the official opencart forums, or seek a freelancer to do the job for you.
