Registration error

When visiting my registration page a popup Not Found error, and then blank fields on the registration form itself.

Does this error appear with the default opencart theme enabled ?

No, the error does not appear using the default Opencart theme. It works fine. Only with the Shoppica theme is this a problem. I don’t have any other modules installed, etc. This error appears when viewing the account/register page, as well as checkout/cart and checkout/checkout pages. I have tried to re-upload all Shoppica files without success.
