i install the shopica 2 theme (first time)

then i upload all file including the index.php in the additional folder , activate the shopica 2 module then i also install sample data ,all thing is set and good when i test the store front , however there is a problem when i try to create a new account  , after i fill up all info and second step it show the error code as below :
